Direct Primary Care & Membership

How the Aquarian membership works, what it costs, and what's included.

Direct Primary Care is a membership-based model where you pay a flat monthly fee to your practitioner instead of billing insurance for every visit. At Aquarian Health, DPC covers unlimited in-office and telehealth visits, same-day messaging with your provider, wholesale labs, and primary care without copays or claims. It's primary care the way it used to be — just faster, with modern tools.

Individual membership starts at $109/month. Couple plans are $180/month for two adults. Family plans are $240/month for up to four members. Every tier includes unlimited visits, same-day / next-day appointments, 24/7 provider messaging, wholesale lab pricing, and in-office procedures — with no copays, claims, or hidden fees.

No. Direct Primary Care is not insurance — it's better primary care. Most Aquarian members pair their DPC membership with a high-deductible catastrophic health plan or health share to cover hospitalizations, surgeries, and specialist referrals. The combination usually costs less than traditional insurance while delivering dramatically better day-to-day care.

Unlimited in-office and telehealth visits, same-day or next-day appointments, 24/7 text messaging with your practitioner, wholesale lab pricing, preventive care, chronic disease management, in-office procedures, care coordination, and referrals. Hormone therapy, peptides, and weight-loss programs can be added at any time at member-only rates.

Yes. Aquarian memberships are month-to-month with no long-term contract. You can cancel at any time for any reason. If you move, need a pause, or just want out, we'll handle it without friction. Most members stay because the care is worth it, not because they're locked in.

Traditional concierge practices charge $2,000–$10,000+ per year on top of still billing your insurance. Aquarian's DPC is a single flat monthly fee with no insurance billing at all — more access, fewer surprises, a fraction of the cost. You still get the same-day access, direct messaging, and unhurried visits that concierge promises.

In most cases, yes — HSA and FSA funds can typically be used for Aquarian memberships, labs, and medical services, though policies vary by employer and plan administrator. We recommend confirming with your HSA/FSA provider. Aquarian provides itemized receipts that make reimbursement straightforward.

Yes. Unlike traditional primary care, where a fully booked schedule means a three-week wait, Aquarian providers hold open slots every day for established members. Same-day or next-day access is standard — not a perk — because fast access is the whole point of the DPC model.

Locations & Telehealth

Where our in-person clinics are and what's available remotely.

Aquarian Health has two in-person clinics in Florida: Aquarian Clinic Jacksonville at 920 Margaret St, Jacksonville, FL 32204, and Aquarian Clinic Tallahassee at 1350 Market St, Suite 202, Tallahassee, FL 32312. DPC members choose a home base clinic when they enroll. You can be seen at either Florida location as needed.

Yes, for Direct Primary Care. Because DPC is an in-person relationship with a specific practitioner, every member chooses a Jacksonville or Tallahassee home base at enrollment. You can still message your provider and do telehealth visits from anywhere, but your home base is where your doctor-patient relationship lives.

Yes. If you move, change jobs, or just want a different location, we'll transfer your home base between Jacksonville and Tallahassee with no re-enrollment and no fees. Your chart, your history, and your care team context travels with you.

Yes. DPC members have unlimited telehealth visits included with their membership. Most primary care issues — medication changes, lab review, follow-ups, minor illness — can be handled virtually. Your home base practitioner sees you for physicals, procedures, and anything that needs hands-on care.

Yes. Aquarian Health's telehealth TRT program is available in all 50 states. Labs are drawn at a local LabCorp or Quest near you, the consult is conducted via video, and medication is shipped directly to your door from our compounding pharmacy. Most patients never set foot in a clinic.

Men's Hormone Therapy (TRT)

Testosterone optimization, programs, and what to expect.

Classic symptoms include low energy, poor sleep, stubborn belly fat, low libido, brain fog, and workouts that don't produce results anymore. The only way to confirm is a comprehensive blood panel including total testosterone, free testosterone, SHBG, estradiol, and LH/FSH. Aquarian's initial TRT workup covers all of it.

Aquarian's telehealth TRT program starts at $198/month and includes medication, supplies, provider oversight, and quarterly lab reviews. The Men's Hormone Care Plan at $322/month bundles TRT with Direct Primary Care, all labs included, and unlimited provider messaging — most members save $274/month versus paying separately.

Most commercial insurance plans don't meaningfully cover TRT for optimization — they often require testosterone levels below a narrow cutoff before approving coverage, and prior authorizations can take weeks. Aquarian's cash-pay TRT program is typically less expensive than running testosterone through insurance, and treatment can start within days of your initial labs.

Most men notice energy and mood improvements within 2–4 weeks. Libido and sleep usually improve by weeks 4–6. Body composition changes — fat loss, muscle gain — show up over 3–6 months. Full optimization of free testosterone, estradiol, and related markers is typically dialed in by month 3.

No. Most Aquarian TRT members self-administer weekly testosterone injections at home after a brief training with your practitioner. We also offer cream, pellets, and other delivery routes when clinically appropriate. The goal is whatever protocol fits your life and delivers stable levels.

Testosterone cypionate and testosterone enanthate are two ester forms of injectable testosterone with nearly identical clinical profiles — both are long-acting, dosed weekly or twice-weekly, and produce equivalent results for the vast majority of patients. Cypionate is more commonly prescribed in the US. Your practitioner will match the form to your response and lab values.

Women's Hormone Therapy (HRT / BHRT)

Bioidentical hormones, perimenopause, and menopause care.

Bioidentical hormone replacement therapy uses hormones — estradiol, progesterone, and sometimes testosterone — that are molecularly identical to what your body naturally produces. Aquarian's BHRT protocols are physician-designed and personalized to your labs, symptoms, and goals. Delivery can be oral, transdermal, injectable, or pelleted depending on what fits your life and your chemistry.

Women in perimenopause, menopause, or post-menopause experiencing hot flashes, sleep issues, mood changes, low libido, brain fog, weight gain, or joint pain are often excellent HRT candidates. Women with surgically-induced menopause also benefit significantly. Aquarian reviews your full medical history, symptoms, and labs before recommending a protocol.

HRT starts at $400/month for medication and provider oversight. The Women's Hormone Care Plan at $480/month bundles HRT with Direct Primary Care, quarterly labs, and unlimited provider messaging — most members save $400/month compared to paying for each piece separately.

Sleep and mood improvements often show up within the first 1–2 weeks. Hot flashes and night sweats typically improve by weeks 2–6. Energy, libido, and body composition benefits build over 2–4 months. Your practitioner will re-check labs and adjust dosing at 8–12 weeks to dial in your protocol.

Medical Weight Loss (GLP-1)

Semaglutide, tirzepatide, and physician-supervised weight loss.

Aquarian prescribes semaglutide (the molecule in Ozempic and Wegovy) and tirzepatide (the molecule in Mounjaro and Zepbound) as part of our physician-supervised weight loss program. Both are delivered via compounded formulations from a US-based 503A pharmacy with quick shipping, and both are overseen by an Aquarian practitioner throughout treatment.

Aquarian's compounded semaglutide and tirzepatide programs are typically a fraction of branded GLP-1 retail pricing, which can run $900–$1,300+ per month without insurance. No. Aquarian's weight loss program is a medical, physician-supervised program for adults who want to lose weight — you don't need a diabetes diagnosis. A candidate typically has a BMI ≥ 27 with weight-related concerns, or a BMI ≥ 30. Your practitioner reviews your labs and history to confirm you're a safe candidate.

Compounded semaglutide and tirzepatide from a reputable US 503A pharmacy use the same active molecules as branded versions and are prepared under state and federal oversight. Aquarian partners with a licensed US pharmacy that produces sterile, third-party-tested compounded GLP-1s. Your medication ships directly to you with instructions from your practitioner.

Clinical studies show average weight loss of 15–20% of body weight over 12 months on semaglutide or tirzepatide at therapeutic doses, when combined with diet and activity changes. Individual results vary based on starting weight, protocol, and lifestyle, but most Aquarian members see meaningful changes within the first 4–8 weeks.

GLP-1 medications work by reducing appetite and improving metabolic signaling, but pairing them with basic protein-forward nutrition and regular movement dramatically improves results and protects lean muscle mass. Your Aquarian practitioner provides specific protein, strength, and activity targets — no fad diets, just what actually works.

Standalone GLP-1 weight loss plans are 100% remote — virtual check-ins with your provider and medication shipped directly to your door. If you want in-person visits at our Jacksonville or Tallahassee clinics, add a Direct Primary Care membership or choose a care plan that bundles weight loss with DPC. Care plan bundles on the Men's and Women's Hormone pages already include DPC, so in-person access comes built in.

Peptide Therapy

BPC-157, CJC/Ipamorelin, NAD+, and peptide protocols.

Aquarian offers physician-supervised peptide protocols including BPC-157 (tissue repair and recovery), CJC-1295 / Ipamorelin (growth hormone support), Tesamorelin (visceral fat reduction), MOTS-c (metabolic health), and NAD+ (cellular energy). All peptides are sourced from reputable US pharmacies and prescribed based on your goals, labs, and history.

Some peptides are FDA approved (like tesamorelin for HIV-associated lipodystrophy); others are prescribed off-label or compounded by licensed US pharmacies. Aquarian only works with reputable pharmacies and prescribes peptides under appropriate medical supervision with regular lab monitoring. Your practitioner will explain the regulatory status of any peptide they prescribe.

Hormones (like testosterone or estradiol) are direct replacements for endogenous compounds your body already produces. Peptides are signaling molecules — short chains of amino acids that tell specific systems in your body to do more of what they're already designed to do: repair tissue, release growth hormone, burn visceral fat. Peptides are typically more targeted and short-course.

Labs & Bloodwork

What we test, where labs are drawn, and how results work.

Aquarian's initial workup typically includes a comprehensive metabolic panel, complete blood count, lipid panel, HbA1c, vitamin D, full thyroid panel (TSH, free T3, free T4), and hormone markers appropriate to your goals (total and free testosterone, estradiol, SHBG, LH/FSH for men; estradiol, progesterone, LH/FSH, testosterone for women). The exact panel is personalized to your program.

Yes. If you have recent (typically within 90 days) comprehensive labs from another provider, your Aquarian practitioner can often use them to start planning your protocol. Some programs still require confirmatory labs, but we'd rather use current data than make you pay for tests you've already done.

Yes. Aquarian offers wholesale in-house lab pricing at our Jacksonville and Tallahassee clinics, and we can also order labs through LabCorp or Quest locations convenient to you — especially useful for telehealth members across the US. Results flow directly into your chart and your practitioner reviews them with you.

Most Aquarian members on hormone therapy or peptides are retested at 8–12 weeks after starting a protocol, then every 3–6 months once stable. Weight loss members typically retest metabolic markers quarterly. DPC members on preventive care get annual comprehensive panels. Your practitioner sets a cadence based on your specific program.

For Direct Primary Care, start at our enrollment page and pick your home base clinic — Jacksonville or Tallahassee. For telehealth TRT, weight loss, or hormone programs only, book your first consult directly and we'll guide you through labs and enrollment. Most members complete enrollment and intake in under 15 minutes.

Aquarian Health does not bill insurance for memberships, TRT, HRT, weight loss, or peptide programs — this is what lets us keep prices transparent, access fast, and visits unhurried. Some labs and prescriptions can still be processed through your insurance or HSA/FSA. We provide itemized receipts for any out-of-network reimbursement you'd like to pursue.

Most Aquarian memberships and programs are billed monthly by default, with no long-term contract. Annual pre-pay options are available for members who prefer to handle it in one transaction. Billing is handled securely through our membership platform and auto-renews until you cancel.

No. Every Aquarian membership is month-to-month. You can cancel any time without penalty. We think the quality of care should keep you here, not a contract.
